The purpose of this order is to outline procedures for the reporting, and following-up of offenses and incidents coming to the attention of the Department. This order consists of the following parts:

PART I

 

A. Definition.

 

     1.   Serious Crimes - homicides, rapes, critical injury assaults,           rob­beries, burglaries when there is physical violence or a loss               in excess of $5,000, bombings and arsons.



     2.   Unusual Incidents - deaths other than homicides which fall under           the statutory jurisdiction of the Medical Examiner’s office,           missing persons, bomb scares and natural disasters.

 

     3.   Field Report Writing Handbook - The official publication of the               department specifying policy, procedures, and guidelines for the       preparation, filing, and indexing of reports. This publication               has the force of a general order as it pertains to the field               reporting system.

 

     4.   Industrial Accidents - accidents causing personal injury and/or               property damage, exclusive of home and traffic accidents, which              are the result of occupational activity.

 

B.   Reporting Requirements.

 

     1.   All reports prepared in the field shall be classified on the           basis of elements present which establish the violation of                  applicable statutes in ef­fect for the District of Columbia,               i.e., D.C. and U.S. Criminal Codes.

 

     2.   Incidents In which multiple offenses have occurred shall be             classified to reflect only the most serious offense, as defined               by penalty in the D.C. Code. Remaining offenses shall be               described in the narrative portion of the report as reported by            the complainant.

 

     3.   It shall be the responsibility of the first member on the scene               of all police incidents, regardless of his assignment, to make               the necessary reports unless directed otherwise by an official           or the dispatcher, Communications Division. The only exception               shall be when compliance would interfere with the investigation           of the criminal case or defeat the ends of justice.

     4.   Whenever members prepare reports on the scene Involving serious               offenses or unusual Incidents, they shall, through the                  dispatcher, Communications Division, request a district                 official to respond to the scene of the offense or incident. The       reporting member shall be responsible for making the necessary               notifications. The member preparing the report of a serious               crime or unusual in­cident shall record the name and                     organizational element of the supervising official responding to      the scene, or the station clerk notified, in addition to any             other notification made.

 

     5.   Radio units shall not cruise with complainants unless directed               to do so by proper authority, or there is an Instance when the           suspect may be In the immediate area and delay in pursuit                    would greatly delay chances of apprehension, or in circumstances      when there is a hostile crowd or any unusual situation when the           safety of the complainant or member is in question.

 


     6.   In Instances when members leave the scene with complainants,           they shall contact and advise the dispatcher of their location.               If the report should be handled by any other unit, other than               the unit originally assigned the call, the dispatcher shall be           notified, and the central complaint number shall be turned over             to the member preparing the report(s).

 

     7.   Members initiating reports shall not be relieved from duty until           all reports executed by them during their tour of duty have been           submitted and corrected as necessary.

 

     8.   Each member initiating a field report shall sign the report in               the appropriate space on the form. Unless specific authorization      has been obtained from the reporting member, no member--other             than the reporting member—-shall sign the field report.

 

C.   Reporting Offenses/Incidents Occurring In Another District.

     When members of the force are dispatched to the scene of an incident      outside their assigned district or when an incident which occurred      outside their assigned district Is reported to them, they shall      prepare the required report. They shall obtain a central complaint      number from the Communications Division, and the report shall be      forwarded through normal channels to the Field Reports Section,      Identification and Records Division. In addition, the reporting      officer shall telephone all information surrounding the incident to      the station clerk of the district in which the event took place. The      name and unit of the officer re­ceiving the information shall be      entered In the narrative portion of the PD Form 251.  The element to      which the resorting member is assigned shall be      responsible for      transmitting the report over teletype when appropriate.

D.   Reports Requiring Response to Hospitals or Clinics.

 

     1.   Members initially dispatched to take reports of sick or injured               persons at locations other than hospitals or clinics shall,               prior to responding to a hospital or clinic located outside of            their assigned organizational element for the purpose of               obtaining information to complete the necessary report, con­tact               a supervisory official of the organizational element to which              they are as­signed for authorization.

 

     2.   Reports requiring information from hospitals or clinics for             com­pletion may be turned over to supervisory officials for                  subsequent completion by station personnel.

 

     3.   When members are dispatched to clinics and hospitals to                 investigate complaints they shall prepare the necessary reports,           sign and date the pa­tient’s chart, when authorized by the                   hospitals, and make any necessary notifica­tions if there is any           reason to believe the subject will be admitted to the hos­pital.



     4.   If a complainant Is to be admitted to the hospital, or there is               reason to believe the complainant is to be admitted, the               reporting member shall notify the Communications Division,                    Telecommunications Branch, and give a brief verbal report, to              Include the central complaint number. This will suffice until           the complete report Is transmitted on teletype by the station             clerk receiving the report.

 

E.   Reporting Offenses/Incidents Occurring In a Community Correctional     Facility (Ha1fway-House).

 

     1.   When a member of the force Is dispatched to investigate an                  inci­dent or offense that has occurred within the confines of a             Community Correctional Facility, he/she shall request, through            the dispatcher, that an official from the district In which the               facility is located respond and be on the scene while the                    necessary reports are prepared.

 

     2.   Upon completion of all required reports, the reporting member           shall telephone the Major Violators Section, Criminal                       Investigations Division, and give to a member of this branch a             brief account of the incident or offense. The reporting member             shall then include the time, date and person within the Major              Violations Section notified in the narrative portion of his/her            report.

F.   Reports by Telephone.

 

     1.   Reports may be taken by telephone from citizens, regardless of               the citizen’s place of residence, If such complaints meet the           reporting criteria as contained in General Order No. 401.10               (Telephone Reporting Unit). These reports shall be taken and               processed by the Telephone Reporting Unit in accordance with               General Order No. 401.10.

 

     2.   Citizens who telephone this department to report a crime or             Inci­dent which does not meet the criteria for receiving reports               by telephone, as con­tained in General Order No. 401.10, shall be      advised that one of the following actions (whichever Is                 applicable) will be taken:

 

          a.   Persons telephoning from locations in the District of                        Columbia shall be advised that a mobile unit will be                         dispatched to take the report.

 

          b.   With the exception of those complaints involving                             circum­stances as set forth in the paragraph below, persons                 tele­phoning from locations outside the District of Columbia           shall be advised to return to the District of Columbia and                make the report. Should the distance involved make this                    Impractical, the complainant shall be advised to forward a                    formal letter containing all the details surrounding the                   incident, to the Director, Communications Division, 300                Indiana Avenue, N.W., Washington, D.C. 20001. The Telephone           Reporting Unit shall then prepare the proper report and                   make the necessary distri­bution of the report.

 


          c.   When the following factors are present, persons residing               out­side the District of Columbia, but within a 25-mile                  radius of the U.S. Capitol Building, shall be advised by an               official of the Communications Division that an                              investigator will be dispatched to take the report:

 

              (1)  The complainant has been the victim of a violent crime                    while in the District of Columbia.

 

              (2)  The complainant is physically unable to return to the                         District of Columbia.

 

          D.   Under special circumstances, and upon approval of the                        Director, Communications Division, or the night supervisor,             an investiga­tor may be dispatched to a location beyond the                   25 mile radius for the purpose of taking a report.

G.   Occurrences in Other Jurisdictions.

 

     1.   In cases in which persons are transported to the District of           Columbia facilities for treatment of injuries received in                   another jurisdiction as a result of a traffic accident, the               commission of criminal offenses, or other incidents requiring a             police investigation, the member responding to the hospital           shall prepare a PD Form 251 (Event Report).

 

     2.   When persons are admitted to hospitals in the District of                   Columbia as the result of a traffic accident that occurred                    outside the District of Columbia, and their condition is listed               as critical or death occurs, the reporting member shall notify            the Hit-and-Run Follow-up Section, Traffic Branch, Special                 Operations Division. If unable to notify the Hit-and-Run Follow-              up Section, the member shall contact the Communications Division           which shall then make the notification.

 

     3.   In the interest of expediting police action in those cases in           part Ig1, the reporting member shall telephone the jurisdiction               involved and provided all necessary information. Telephone calls         involving toll charges shall be handled in accordance with                    General Order 302.3.

 

H.   Occurrences in Park Police Jurisdiction.

 

The U.S. Park Police are charged with the responsibility to investigate and report all incidents and offenses on Federal Park property, except for deaths, which are reported and investigated by this department. Members of the force shall promptly notify a member of the U.S. Park Police of any incident or offense coming to their attention. In addition, the required Metropolitan Police Department reports prepared dealing with deaths shall reflect the time and date the U.S. Park Police were notified, including the name of the official or officer receiving the information.

 


I.   Reporting of Stolen Automobiles and Recovered Automobiles.

 

     1.   PD Form 251 (Event Report) shall be used in reporting auto                  thefts, lost autos, tampering with autos, and stolen autos from               other jurisdictions recovered in the District of Columbia                (interstate recoveries).

 

     2.   PD Form 252 (Supplement Report) must contain the same central           complaint number as the original report.

 

     3.   PD Form 81 (Property Return) shall be prepared and retained as               the impounding organizational element’s record in all cases               involving impounded motor vehicles (e.g., towed ir1 from traffic            accidents, parking law violations, autos taken from prisoners as       personal property, etc.)

     4.   In instances where the person reporting the vehicle stolen is           not the owner (e.g., parking lot or service station attendants),           the reporting member shall obtain all possible identifying                  information relative to the auto, transmit this information in            the form of a “Lookout for Suspected Stolen Auto,” to the                    Communications Division, and leave sufficient information with             the reporting person to ensure that the owner can later contact            the department to file the necessary’ report.

 

     5.   The Telecommunications Branch, Communications Division, shall be           contacted by telephone and given recovery information on all           vehicles recovered by a member of this department. The teletype               operator will Verify the call while the member is still on the           line. Upon completion of verification, the vehicle lookout shall         be canceled in WALES and NCIC from the information given to the           Telecommunications Branch over the telephone. The local teletype      lookout shall be canceled only upon receipt of the cancellation            teletype message from the recovering officer’s unit. The                    telephone call and subsequent cancellation does not eliminate           the need for the recovering member to submit a PD Form 251 or               252, as applicable.

 

     6.   When vehicles are recovered by members of the force, the                recovering member shall immediately notify the owner or the             person who filed the report. If members are unable to contact           the owner or the reporting party, a notation of the attempt             shall be made on PD Form 251 or 252 and brought to the personal               attention of the station clerk who shall be responsible for               completing the notification. In instances where the owner or               person reporting the vehicle stolen resides outside the                 District, and cannot be reached by telephone without expense to            the District of Columbia. the recovering member shall make the           long distance notification in compliance with General Order               302.3 (Department Telephones).

 

     7.   In a case where a motor vehicle is recovered which was stolen in           another jurisdiction and in other cases where interstate                transportation is involved, the recovering member shall notify               the Auto Theft Desk and enter on the report the name of the               person notified along with the time and date of notification.           The Auto Theft Desk shall also be notified in all cases of                recovery of stolen motor vehicles. 

 

     8.   When preparing PD Form 251 or 252 on recovered vehicles which           have been stripped of parts or accessories, Indicate in Item 30               on PD Form 251 or item 10 on PD Form 252 if the motor or the               transmission was taken. If either of these items is missing,               record the manufacturer’s vehicle Identification number as re­            corded on the metal plate attached to the vehicle.

J.   Foot/Scooter Patrol Members Taking Reports.

 

     1.   When members assigned to foot or scooter patrol are In receipt               of original complaints In which an offense or Incident report is          required, and they do not have to physically leave their beat,             they shall record the necessary In­formation In their notebook,              contact the patrol signal system officer, and re­quest a sergeant      to respond to their location. When the sergeant arrives, they     

     2.   This type of report Is limited to the following:

 

          a.   Animal bite.

          b.   Destroying property.

          c.   Thefts (all types).

          d.   Lost property.

          e.   Robbery (PBS and pickpocket).

          f.   Sick or injured person (only those occurring on public                  space).

          g.   Simple assault.

          h.   Stolen bicycle.

 

     3.   Further exceptions to Item 01 above are as follows:

 

          a.   Members shall not physically leave school crossings,                         traffic posts, or their assigned beats to take this type of            report; however, they shall assist the complainant in any                way pos­sible.

 

          b.   Scooter members assigned only to crime patrol shall also be                exempt from taking reports, but shall assist the                             complainant by contacting a patrol signal system officer                 and requesting a uniform patrol car to be dispatched for                    the complainant.

 

K.   Evaluation of Lost or Stolen Property.

 

     1.   When executing a PD Form 251 or 252 in which lost or stolen             property is being reported, the reporting member shall comply           with the following guidelines In assigning a fair and equitable               valuation to the property.

 

          a.   Fair Market Value.

              The fair market value shall be used for articles which are                    subject to depreciation because of wear and tear, age, or                 other factors which cause the value to decrease with use.                    (For example, clothes; household articles, etc.).

          b.   Wholesale Cost to the Merchant.

              The wholesale cost to the merchant shall be used for goods                    stolen from retail establishments, warehouses, etc. In                    other words, use the dollar value representing the actual                  cash loss to the victim without any markup or profit added.

 

          c.   Complainant Value

              The complainant’s evaluation shall be used on Items Such as

              jewelry or , watch and other similar goods which decrease                    in value slightly or not at all with use or age.

 

          d.   Replacement Cost.

              The replacement cost or actual cash cost to the victim                  shall be used for new or almost new Items, e.g., clothes,                    auto ac­cessories, bicycles, etc.

 

     2.   Instruments such as traveler’s checks, personal checks, money           orders, stocks and bonds, etc., shall be reported as a theft or               loss, and a mone­tary value of twenty-five cents shall be               assigned to each Item, or items so re­ported.

 

     3.   All Instruments such as bonds payable to the bearer, e.g., U.S.               Government Bonds, U.S. Treasury Bonds, etc., shall be valued at            current market price at the time of theft or loss.

 

     4.   Food stamps are valued at face value.

 

     5.   Members of the force shall, when completing reports on lost or               stolen property, enter In the blocks provided the com~ia1nant’s           estimate of the value of the property and their evaluation of           the property, even though it may vary from the complainant’s           estimate.

 

     6.   In instances where more than one Item is reported lost or                   stolen, the property shall be itemized and each item given a               police department value.

 

     7.   When property is recovered, the value originally reported shall               be assigned unless it is obvious that the value has depreciated.

 

L.   Industrial Accidents

 

     1.   Examples of industrial accidents are:

 

          a.   The collapse of a scaffold on a construction, repair, or               painting job.

 

          b.   The fall of an elevator.

          c.   Employees injured in the operation or maintenance of                         machinery, construction work, loading or unloading of                         trucks.

 

          d.   Contact with electricity resulting in injury, fires, burns,               etc., from occupational activity; or

 

          e.   Any type of accident resulting from a person’s employment.

 

     2.   Members of the force who learn of industrial accidents shall           prepare a PD Form 251 and as soon as practicable notify the             Occupational Safety and Health Division, D.C. Department of             Employment Services of the following:

 

 

          a.   Location of the Accident.

          b.   Time of the accident.

          c.   Type of accident.

          d.   Any other pertinent information required by     the                          Occupational Safety and Health Division.

 

The date, time and person notified shall be entered on PD Form 251,together with the name of the member who made the notification.

 

The Occupational Safety and Health Division shall be notified between the hours of 0800 and 1600, Monday through Friday, by telephone. The division will then make its own investigation.

 

Part II

 

A.   Station Clerks.

 

     1.   A station clerk, upon receiving notification that a crime or           incident has occurred in the district either from the dispatcher           or a member of another district, shall obtain the pertinent             information, and, in serious crimes or unusual incidents, notify           the official then in command.



     2.   The station clerk receiving a crime or incident report from an               officer in another district shall be responsible for preparing a        duplicate, information only copy, for his district investigators      and Crime Analysis Section. In addition, the incident shall be           listed on the occurring district’s PD Form 93. The notation           “duplicate report” shall be written in bold print at the top of              the PD Form 251 and this report shall not be forwarded to Staff           Review. The duplicate, information only, report shall be filed             in the district for a 90-day period.

     3.   The station clerk working the 0800 to 1 600 tour shall, upon           receipt of a formal letter detailing an incident or offense,           forward the letter in a separate envelope to the Telephone                  Reporting Unit, Communications Division, for preparation of the           proper report.

 

     4.   Station clerks shall be responsible for the completion of                   applicable teletype message reports, as directed by supervisory             officials, and for effecting the notification necessitated by              the completion of these, and any other reports.

 

     5.   When an industrial accident comes to the attention of a member,               other than during hours that the Occupational Safety and Health             Division is open, an appropriate station record shall be made            and the day Station Clerk shall report the incident to this               division as soon as possible on the next business day.

 

B.   Providing Reproduced Copies of PD Form 251 (Offense/Incident      Reports).

     The following procedures shall apply to the processing of requests      for reproduced copies of PD Form 251 (Offense/Incident) reports.

 

     1.   There are two methods for providing reproduced copies of PD Form           251 reports:

 

          a.   Those submitted through the mail.

          b.   Over-the-counter requests.

 

     2.   All requests for copies of PD Form 251 (Offense/Incident Report)           shall be referred to the Identification and Records Division.           Persons requesting copies of PD Form 251 shall be advised that               such requests can be made by mail or in person.

 

Under no circumstances shall personnel, other than those authorized by the Director, Identification and Records Division, accept fees of any kind for copies of PD Form 251.  All requests shall be referred to the Identification and Records Division.

 

Mall requests for PD Form 251 shall be forwarded to the Director, Identification and Records Division, for processing: Requests for PD Form 251 shall be accompanied by check or money order and made payable to the Collector of Taxes, D.C. In those instances where cash has been received through the mall, It will be turned over to the supervisor handling such requests In the Identifica­tion and Records Division and processed Immediately. Such fees received with mall requests shall be deposited each workday with the Collector of Taxes by a designated person from the Identification and Records Division.

     5.   Over-the-counter payments made by check or money order shall be               processed in the same manner as mail requests. Persons                  requesting to make pay­ments in cash shall be directed to make               such payments to the Collector of Taxes, Room 1140, Municipal               Center. Upon validation of the receipt by the collector, the               person shall return to the Identification and Records Division             and exhibit the paid receipt in order to receive the copy of the           PD Form 251.

 

     6.   In those instances where a thorough search of the files of the               Identification and Records Division falls to reveal the                 existence of a report of the requested offense/incident report             alleged to have occurred, a Certificate of Search shall be issued          by the Identification and Records Division, and the appropriate             fee charged.

    

     7.   Fees collected for the search shall be processed and deposited               each day with the Collector of Taxes, D.C., in the same manner             prescribed for the collection of fees for reproduced copies of               PD Form 251.

 

     8.   No cash shall be accepted by personnel of the Identification and           Records Division except In those instances where It has been           received through the mail. Therefore, any request for a copy of               a PD Form 251, made in person and accompanied by cash, must be            made between 0900 and 1500 hours on Monday through Friday,                    excluding holidays. Such requests shall be handled In accordance         with procedures outlined in Part 11B5 above.

 

     9.   Mall requests for copies of PD Form 251, accompanied by                 payments, received on Saturdays, Sundays, or holidays and                   processed by the Identification and Records Division, shall be            held until the following workday for deposit with the Collector               of Taxes, D.C.

 

C.   Distribution and Files.

 

     1.   Two (2) reproduced copies shall be made of all offense reports               and all supplements to offense reports.

 

The original shall be forwarded to Staff Review Section, identification and Records Division, within 24 hours. In the event of a holiday or weekend, the original shall be    forward­ed on the next business day with the morning papers.

 

 

If the status of the offense is Closed by Arrest,’ the    cor­responding PD Form 252 shall be attached to the PD Form 251 before It Is forwarded to Staff Review Section.

          c.   The first reproduced copy shall be forwarded to the                     district detective’s office.

 

          d.   The second reproduced copy shall be filed in the element’s                    90—day file. This copy shall be used by the station clerk                   to make the PD 93 (24-Hour Crime Report) and by the crime                    analy­sis officer to compute crime statistics.

 

     2.   One (1) reproduced copy shall be made of all incident reports           and all supplements to incident reports. (NOTE: the category           incident report” in­cludes missing person reports.)

 

          a.   The original shall be forwarded to Staff Review Section                 Identification and Records Division, within 24—hours. In               the event of a holiday or weekend, the original shall be               forward­ed on the next business day with the morning                     reports.

 

          b.   The reproduced copy shall be filed in the element’s 90-day

              file.

 

D.   Auto Theft Desk.

 

     1.   In instances where the owner or person reporting a vehicle                  stolen resides outside the District of Columbia and cannot be            reached by telephone with­out expense to the District of                 Columbia, the Auto Theft Desk, upon receiving the proper                notification, shall be responsible for notifying the owner or            person re­porting the vehicle stolen.

 

     2.   In cases involving interstate stolen vehicles, the Auto Theft           Desk, upon receiving the proper notification, shall be                  responsible for notifying the Federal Bureau of Investigation.

 

E.   Staff Review Section, Identification and Records Division

 

     1.   The Staff Review Section shall:

 

          a.   Reclassify all offenses and incidents which were classified

              Incorrectly.

 

          b.   Return a copy of any report needing to be corrected to the                    original reporting element for correction.

 

     2.   An original corrected report shall then be forwarded to the             Staff Review Section within 7 days from the date the report was               returned for correc­tion.

PART III

 

A.   Communications Division.

 

     1.   An official of the Communications Division shall, upon receipt               of a complaint as outlined in part 1F2, ascertain the                        complainant’s name, address, telephone number, the police                  jurisdiction In which the complainant Is located and the                 location in the District of Columbia where the crime occurred.

 

          a.   A Communications Division official shall advise the                     complain­ant that an investigator will be dispatched to                  Investigate the complaint.

 

          b.   The official shall then dispatch a plainclothes district               in­vestigator from the district in which the crime occurred,               after having notified and cleared the assignment through                the senior district official on duty.

 

     2.   The Communications Division official shall then place a call to               the police jurisdiction to which the member(s) Is responding and       advise the police dispatcher that our member(s) is responding to           such an address and for what reason. If the member(s) responding           for the investigation is not familiar with the address he shall               advise the dispatcher who shall request directions or have a               member of the police jurisdiction in which the victim is located       meet our member(s) at a prearranged location and escort him to            the address.

 

     3.   if a complainant should request a report to be made, and he/she               lives outside the 25—mile limit, any response by members of the           department shall be approved by the Director, Communications               Division, or the night supervisor.

 

     4.   Upon receipt of a telephone call from a member who has recovered           an auto stolen in this jurisdiction, ,a member of the                       Telecommunications Branch shall:

 

          a.   Clear the lookout information in WALES and NCIC,

 

          b.   Place the teletype copy of the lookout and recovery                     infor­mation in a suspense file,

 

          c.   Cancel the local teletype lookout only upon receipt of the                    cancellation teletype message from the recovering officer’s               unit, and

          d.   Transmit a daily list of vehicles that have been recovered,               but not officially cancelled over the local teletype system               during the previous 24 hour period.

 

     5.   Upon receipt of a telephone call from a member who has recovered           an auto stolen In another jurisdiction, a member of the                 Telecommunications Branch shall:

         

          a.   Place a ‘LOCATE’ in the NCIC entry of the stolen vehicle,                    and

 

          b.   Send a message over the National Law Enforcement                             Telecommuni­cations System to the originator of the stolen                  report to con­firm the NCIC entry.

 

B.   Director Identification and Records Division.

 

The Director, Identification and Records Division, shall be re­sponsible for ensuring that reporting elements are notified of all reports re­classified by the Staff Review Section.

 

 

When field reporting elements disagree with a classification of a report by the Staff Review Section, and the matter can not be satis­fied or resolved, field reporting elements may appeal the classification to the Staff Review Section of the Director, identification and Records for final resolution.
